Each of these encounters may involve an office visit, an imaging procedure, a drug injection, a cystoscopy, and a specimen from a path lab, all of which will need to be coded, sequenced, and documented properly before a payment is made by a payer. That is why practices that employ accurate urology billing services<\/a> always get more of what they deserve. <\/p>\n\n\n\n The 2027 cycle raises the stakes even further. New CPT codes, a fresh ICD-10-CM release, and proposed Medicare payment cuts are all arriving within a few months of each other. The five tips below, shared by the coding team at CureCloudMD<\/a>, give urology coders and billers a practical, code-level approach to protecting reimbursement in this environment.<\/p>\n\n\n\n Urology practices are facing change on three fronts at once. The AMA\u2019s CPT 2027 code set brings 453 editorial changes, including 299 new codes, 74 revisions, and 80 deletions, all effective January 1, 2027, and prostate biopsy is one of the areas directly affected.<\/p>\n\n\n\n Diagnostic codes<\/strong>: FY 2027 ICD-10-CM update has added 190 new codes and removed 30 codes and changed four codes effective October 1, 2026. So are payments, too: CMS anticipates that Medicare payments to urology will be cut by approximately 2% under the proposed rule for CY 2027. <\/p>\n\n\n\n Each undercoded claim and each denial will cost more when reimbursement amounts are decreasing. Therefore, the most effective tool for maximizing revenue in a urology practice is to code accurately, and it’s not about the billing speed. <\/p>\n\n\n\n A cystoscopy is the cornerstone of urology procedure coding and one of the most common claim coding errors. The main rule is, diagnostic cystoscopy is always included with a surgical cystoscopy and should always be coded as the most complete service performed. <\/p>\n\n\n\n CPT 52000 (cystourethroscopy, separate procedure)<\/a> applies only when the provider inspects the urethra and bladder without performing any further procedure. The \u201cseparate procedure\u201d designation means it cannot be reported with a more extensive cystoscopy in the same session.<\/p>\n\n\n\n Once the provider adds a therapeutic or diagnostic step, the code changes:<\/p>\n\n\n\n Bladder tumor codes are selected by tumor size, which makes the operative note the deciding factor:<\/p>\n\n\n\n One of these codes can be reported per session. There is some variation between payers regarding how to select; some choose the biggest tumor, others select based on the total size, so check the payer’s written policy and stick to it. CPT 52332 reports cystourethroscopy with insertion of an indwelling ureteral stent. However, 52356 (cystourethroscopy with ureteroscopy and lithotripsy) already includes stent insertion, so 52332 should never be added on the same side.<\/p>\n\n\n\n For bilateral work, check the Medicare bilateral surgery indicator before appending modifier 50. When different procedures are performed on each side, such as lithotripsy on the right and a stent alone on the left, report 52356-RT and 52332-LT, adding XS or 59 when payer edits require it. The proper coding is N13.2, 52356-LT. N13.2 is a specific diagnosis code for hydronephrosis due to renal and ureteral calculous obstruction and contains the obstruction detail payers will want to see. <\/p>\n\n\n\n Prostate biopsy is the single biggest CPT change for urology in 2027, and it follows a complete overhaul in 2026. Practices that copy their 2026 setup into 2027 will see rejections.<\/p>\n\n\n\n In 2026 the legacy code 55700 was already phased out and substituted by a code family for the approach. For 2027, the AUA\u2019s coding review identifies 55706, 55712, and 55713 for deletion, 55707 through 55711 and 55715 for revision, and a new add-on code tied to 55711.<\/p>\n\n\n\n The revised wording, added to AAPC reporting, eliminates the sample-count limits and separates regional, regional plus fusion-targeted and only targeted biopsy patterns, as well as a new +55716 add-on for further targeted lesions. Final descriptors only exist in the official CPT 2027 code book; each number should be checked in the code book prior to the charge master’s upkeep. <\/p>\n\n\n\n The new construction eliminates the use of the word biopsy alone as a support for any code. The procedure note should include the following: <\/p>\n\n\n\n Because MRI-targeted biopsies often require payer approval, build these details into your prior authorization<\/a> requests as well, so the authorized service matches the code billed.<\/p>\n\n\n\n For the prostate biopsy and any prior prostate MRI the diagnosis selected meets medical necessity requirements: <\/p>\n\n\n\n A biopsy claim should never be coded as C61 on a claim when it is suspected. Record the sign\/symptom or abnormal finding that prompted the procedure and change the diagnosis when it is finalized at the time of pathology. <\/p>\n\n\n\n Modifiers determine the payment of a urology claim in full, in part or deny the claim. Modifier 25 is the most important of the modifiers in 2027. <\/p>\n\n\n\n 52000 is a common procedure performed in a urology office that has a 0-day global. The procedure pre-procedure assessment is already described in the procedure payment and the E\/M service is separately billable only when the provider is treating a significant, separately identifiable problem. We have a detailed modifier 25 billing guide<\/a> which explains the documentation standard in detail. <\/p>\n\n\n\n CMS has proposed that when modifier 25 is used with a procedure carrying a 0-, 10-, or 90-day global, the higher-valued service is paid in full and the other is paid at 50%. LUGPA notes this would affect routine urology visits combined with cystoscopy, prostate biopsy, bladder treatments, and catheter procedures. The final percentage may change, but payer scrutiny of modifier 25 will rise either way.<\/p>\n\n\n\n Patient with a history of bladder cancer presents for his routine follow up cystoscopy. The urologist, also during the same visit, suspects that the nocturia has worsened and the stream is weak, and he makes the diagnosis of BPH with LUTS and begins tamsulosin. <\/p>\n\n\n\n The E\/M service is supportable because a new problem was assessed and managed with prescription medication.<\/p>\n\n\n\n Globals for urology surgeries: TURP (52601), Laser Enucleation of the Prostate (52649), Vasectomy (55250) are 90-day globals. In that time, programmers should apply: <\/p>\n\n\n\n Urodynamic and imaging services are often split between the facility and the physician. Append 26<\/strong> when the urologist only interprets the study and TC<\/strong> when only the technical portion is billed. Billing the global code when the hospital owns the equipment is a common audit finding, especially for practices that also work with hospital billing services<\/a> teams on facility claims.<\/p>\n\n\n\n CMS proposes deleting G2211 and replacing it with a modifier that keeps the same descriptor and adds 16% of the E\/M code\u2019s value. Final details will come with the final rule in early November, so E\/M templates should be updated once it is published.<\/p>\n\n\n\n One of the quickest ways to medical necessity denials in urology is to have unspecified diagnosis codes, particularly for imaging, urodynamics, and repeat procedures. Having a current list of common ICD-10 codes<\/a> helps coders to avoid using unspecified codes. <\/p>\n\n\n\n When BPH is present but not causing symptoms, use N40.0; when LUTS is documented, use N40.1. For the related symptoms, N40.1 specifies to provide codes for: <\/p>\n\n\n\n Hematuria codes are separated by type, and payers increasingly expect that detail:<\/p>\n\n\n\n Reserve R31.9<\/strong> for notes that genuinely do not specify the type.<\/p>\n\n\n\n This distinction makes the difference between payable surveillance cystoscopy and non-payable: <\/p>\n\n\n\n If surveillance identifies a recurrence that is fulgurated, switch to the active C67 code and the matching tumor code, such as 52224. If there is obstruction due to hydronephrosis, then N13.2 will include both the obstruction and the calculus, and if there is documented infection (pyonephrosis), N13.6 would be used. <\/p>\n\n\n\n The genitourinary chapter itself gained only two new codes this year. The more useful change for urologists sits in the new K6A category for diseases of the pelvis, which separates prevesical (retropubic) abscess from other pelvic abscesses. Add it to EHR pick lists so providers can find it for postoperative and perivesical infections.<\/p>\n\n\n\n The revenue sources of urology are a big part office based diagnostics and drug administration, and it’s full of bundling and unit traps. <\/p>\n\n\n\n A full multichannel urodynamic study is typically coded as a combination rather than a single code:<\/p>\n\n\n\n Since 51729 already includes the lower-level cystometrogram codes (51726, 51727, and 51728), they should never be reported together. Bill 51798<\/strong> for post-void residual by ultrasound only when it is performed and documented separately.<\/p>\n\n\n\n There are underpayment and overpayment demands that happen in the drug unit: <\/p>\n\n\n\n Always calculate units from the HCPCS descriptor, never from the number of vials or syringes used. CMS proposes a work RVU of 0.90 for 90912 (first 15 minutes of one-on-one perineal or urethral sphincter biofeedback) and a work RVU of 0.50 for 90913 (each additional 15 minutes of one-on-one perineal or urethral sphincter biofeedback). The time spent face-to-face must be recorded in each session as units are based on this. <\/p>\n\n\n\n Post-operative and medication follow-up is the most frequently used telehealth application in urology. Beginning January 1, 2027, CMS will mandate specific modifiers be included on certain claims for telehealth services, including services furnished through a contracted platform or incident to a practitioner’s service.
